DC PACT
DC PACT (Positive Accountable Community Transformation) is a Collective Impact coalition effort of community providers, including social service nonprofits, faith institutions, behavioral health providers, hospitals, and community health centers, in partnership with multiple District government agencies.

Communications and Development Work Group
The Communications and Development work group is for the leads of communications and philanthropy professionals at each health center to gather and share best practice, strategize, and ensure consistent messaging across our network.

Integrated Behavioral Health (IBH) and Substance Use Disorder (SUD) Work Group
The DCPCA IBH/SUD Work Group is to share best practices and resolve common challenges to advance the IBH model to improve health outcomes. It’s an informal group of peers who can bounce ideas off of each other to advance their practice’s goals. It’s also a vehicle for tackling broader, system-wide challenges for IBH in the District. For example, we collect feedback from members regarding barriers (regulatory, financial, etc.) in the District and can channel this information to appropriate government agencies and partners. The Work Group is comprised of persons responsible for behavioral health from each member health center, including Licensed Clinical Social Workers, Behavioral Health Specialists, Licensed Counselors, and IBH program support staff.
Members of the DCPCA Substance Use Disorder (SUD) Work Group are included on the IBH invitation and welcome to join all meetings given our shared goals. The SUD peer group convenes virtually on an ad-hoc basis primarily to address policy concerns in the District.

Finance and Billing Work Group
The DCPCA Finance and Billing Work Group convenes to share best practices and resolve common challenges to advance financial sustainability for member health centers. It’s an informal group of peers who can bounce ideas off of each other to advance their practice’s goals. It’s also a vehicle for tackling broader, system-wide challenges in the District for cost-effective and value-based healthcare. The group meets monthly in a virtual setting.

Patient Navigators for Cancer Peer Group
The DCPCA Patient Navigators for Cancer Peer Group, begun in May 2018, is a DC-based Peer Group that is open to all navigators/care coordinators/referral specialists in DC metropolitan area who assist patients in various phases of the cancer continuum – from primary prevention to survivorship. In providing a forum for peers to meet and collaborate, we aim to achieve two goals: 1) advance the best practices of patient navigation to improve cancer screening and care; and 2) collaborate to create systems-level solutions to current challenges. This group meets quarterly in a virtual setting.

Emergency Preparedness Task Force (EPTF)
The DCPCA EPTF is a forum for health centers to present emergency preparedness gaps/challenges and implemented improvements, share best practices and resources, identify specific trainings and one-on-one technical assistance needs, participate in webinars with subject matter experts, and receive District-related updates from the DCPCA. DCPCA also ensures each health center has a comprehensive, active Emergency Management Program, and serves as the voice of health centers within the District process for emergency response process. The EPTF is a key opportunity for health center operations and emergency preparedness leads to receive technical assistance and training on building up capabilities for emergency planning and response on a wide range of issues and with an eye on long-term sustainability of these capabilities. This group meets quarterly in a virtual setting.

Human Resources (HR)/Workforce Development(WD) Work Group
The DCPCA HR &Workforce Development Work Group is a forum for health centers to share best practices and resolve common challenges regarding recruitment and retention of health center staff. It’s an informal group of peers who can learn from each other to advance their practice’s goals. It’s also a vehicle for tackling broader, system-wide challenges for retaining quality staff through staff engagement. We invite subject matter experts as guest speakers including those from academia, government agencies, community-based organization, and national healthcare bodies. This group meets monthly in a virtual setting.

Qual IT Work Groups
DCPCA members who are members of the Qual IT HCCN Project have access to additional work groups for sharing best practices, learning from subject matter experts, and coordinating solutions to shared challenges. See below to learn about the WorkGroups available .

Qual IT Clinical Quality Improvement Committee
The Clinical Quality Improvement Committee meets monthly and is focused on using data to improve outcomes. Organizations share aggregate data on standard eCQM and UDS measures across the network. They will also share data, where possible, on population health measures that support value-based payment initiatives as well as non-standard measures for SDOH until standard measures exist.
Who should participate: QI Directors/ Managers/Specialists, reporting staff, Providers, and other interested parties
Qual IT Patient Engagement and Access Committee
The Patient Engagement and Access workgroup meets for one hour on a quarterly basis to develop and expand ways for their health center patients to be more engaged with their healthcare and their ability to access their data. This is a collaborative workgroup that will share their challenges and best practices.
Who should participate: Health center staff that support patient engagement initiatives and patient access HIT tools. This includes EHR Configuration staff, clinical staff, patient engagement staff, and other interested parties.
Qual IT HIT Committee
This is a collaborative workgroup focused on improving data security and risk mitigation, using technology for Information Exchange and using HIT to reduce provider burden. This workgroup will discuss EHR specific topics as well as topics that span all EHRs.
Who should participate: IT/IS Directors/Managers, Security/Compliance staff, Configuration Staff, and other interested individuals.
Qual IT CMO Roundtable/Provider Steering Committee
The CMO Roundtable/Provider Steering Committee convenes virtually each month to allow interested CMOs and Medical Directors to collaborate on clinical and clinical operations topics related to the HRSA HCCN Goals and HRSA HCCN emerging topics. A special focus for this group will also be HRSA HCCN objective on provider burden to help direct how the Qual IT project activities will best support participating members in achieving this objective.
Who should participate: CMOs and Medical Directors or Providers designated by CMO
